If you have a chronic condition, you may be eligible for a Medicare-subsidised Chronic Disease Management (CDM) Plan.

These plans are designed to:

  • Provide access to Medicare rebates for allied health services such as physiotherapy, dietetics, and psychology under a CDM plan
